(comment) In 1966 a series of aviation accidents occurred: five passenger planes crashed, and helicopters involved in the search also crashed twice. Throughout that year, reforms to Japan's aviation safety were demanded. Also, some reports took more than four years to complete, suggesting the investigations ran into difficulties. The All Nippon Airways crash off Haneda, at the time the largest single-plane crash in the world. The pilot switched to visual flight, the cause became unknown, and it became the catalyst for mandatory installation of voice and flight recorders (Makoto Yamamoto).
